Apply for Death Certificate

Documents Required for Certificate:

1. Photocopy of any one of the passport / ration cards / self-certified declaration/voter card / PAN card / driving license / nationalized bank for identity
2. Death Certificate Hospital

OR

In cases where there is no hospital, in such a case, in respect of birth / death certificate to the applicant, the applicant can submit certificate issued by village head / regional councilor / Hon. MP/MLA
